Your Precompose Document Checklist

Completing all the forms you need

This list features all the Precompose resources we’ve made available to help you think about, talk about, and record your wishes for your death care. If you have already spent time thinking about these topics, you may be ready to skip right to signing each form. If you’re still familiarizing yourself, the articles below will provide background and context ahead of making these decisions.

Legal Forms and Information

There are four forms you will need to complete to document your wishes and legally ensure they will be carried out:

1. Disposition Authorization

2. Designated Agent for Funeral Arrangements

3. Vital Statistics

4. Transport Form

Please click to complete and submit your final forms only once. If you start a form but do not complete and submit it, our signature system Zoho Sign may send you email reminders to complete the form. Zoho Sign starts a new form every time you click the “Link to submit,” so you may get reminders even if you have since clicked and submitted another version of the form.

As you complete and sign each of these four forms, be sure to save a copy with your important papers. If you need to change a form, let us know at and we’ll send you a new blank form.
