Once you’ve completed your Precompose contract and initiated payment to become a Precompose member, we encourage you to document your wishes for your future death care to ensure that things go smoothly after you die. If you are not yet a Precompose member, you can join here.

Legal Forms and Information

The resources below are designed to help you consider, discuss, and legally document your death care wishes. You can see a sample version of each document as well as a link to sign and submit your final version online. You will have the option to save and print your completed forms if you choose.

You can also print and complete the forms on paper if you prefer.
