Dear [Name],

I am writing to inform you I have chosen Recompose for my death care. When I die, Recompose will steward my body through the process of natural organic reduction, which is the gentle conversion of human remains into soil. I’ve chosen this process because it is good for the planet and meaningful for me on a personal level.

I’ve appointed [name of your designated agent] to ensure my end-of-life wishes are followed upon my death. They will work with Recompose to ensure my wishes for my death are followed with respect and care.

I have also included the following documents for your records:

  1. Disposition Authorization, which designates my choice of Recompose and wishes for my soil
  2. Designated Agent for Funeral Arrangements, which names the above as my agent
  3. [Optional: Transport arrangements if relevant, proof of Precompose payment]
